Abstract
An assessment of what is known about problems related to enhancing patient transitions from treatment in a regional cancer centre to survivorship, options for addressing these problems, and key implementation considerations. The evidence brief was an input to a stakeholder dialogue on the topic of enhancing patient transitions from treatment in a regional cancer centre to survivorship in the Hamilton Niagara Haldimand Brant community.
